Special Olympics Ontario is seeking nominations for the 2012 Awards. The following awards will be presented:
Male and Female Athlete of the Year
Male and Female Coach of the Year
Volunteer of the Year (non-coach)
Team of the Year
Educator of the Year
President's Award
Please fill out one of the forms attached below (click on the 2012 Awards headline from the home page to access the forms). The forms may be scanned or faxed to Farkhanda Shahid at the Provincial Office: farkhandas@specialolympicsontario.com or 416-447-6336. The deadline to receive nominations is May 15th: don't delay in recognizing deserving members from your community!
